The average time for growth is 6-8 weeks, but may vary based on strain.
Keep your bag at room temperature 68-75 degrees. Ambient light is perfect but no direct sun light. A counter, a shelf, or a closet is a good space to keep a bag.
No that is not mold, it is a very common occurrence. It is called fuzzy feet and will happen when there is not enough air exchange in the bag. It does not hurt your mushrooms or change the potency. Burp your bag more often to lower the chance of this happening.
Mushroom caps are shaped like the top of a lollipop. Underneath the cap is something called a veil, it looks like a thin piece of skin. When that veil breaks or tears away from the cap and show the gills of the mushroom, it is time to pick.
This is called a spore dump, the mushrooms were not picked on time so the gills released spores to start reproducing. Go ahead and pick your mushrooms, they are still good, just not as pretty. If you want you can try to wipe off the black (spores), but not necessary.

That's ok, I tell people to buy a seedling heat mat for your bag. This will help regulate the temperature of you bag to where it needs to be. Just remember to place some cardboard or a towel between the mat and the bag, sometimes the heat in the mat is not consistent and can have hot spots. You don't want your bag getting too hot. You can buy these mats on amazon, or find them at a garden store.
